CGSAA Forms
This form must be used when requesting student transfers to other CGSAA school’s teams. The form must be signed by the Principal of the school which the student’s attend. Each request must be initialed by the representative of the team school.
Acknowledgement that must be signed by all Coach's that they have read and understand and agree to abide by the CGSAA Basketball Rules.
. All players with health exceptions must have a League Participation Waiver signed from their parent/guardian and the Principal of the school and on record with the CGSAA Basketball Commissioner. Copies of the waiver must be brought to each game. The opposing coach should also be informed to save problems down the road.
